Job description

As a Care Coach (Healthcare), you will be responsible for the following duties:

• Support the care team to better integrate and coordinate patients’ care across the integrated care system.
• Collaborate with multi-disciplinary team members to provide comprehensive care and advocate care option and services for patients and families with complex psycho social needs.
• Gain understanding of patients’ needs through active engagement and work with the care team to effect care plan to empower patients and their families and behaviour change.
• Participate in community outreach programmes such as health education and involvement in support workgroups.

Requirements:

• Degree in health-related fields
• Working experience in healthcare or social care sectors preferably in case management
