Connext is hiring a Patient Care Coordinator to support a US-based outpatient practice from the Philippines. You will conduct outreach, pre-visit intake, and medical documentation while coordinating referrals and follow-up activities.

The role handles 20–25 patient encounters daily, supports chart preparation, and maintains HIPAA-compliant records with clinical staff. Strong English, data accuracy, and remote-work discipline are essential.

Qualifications

• Minimum two years in customer service or related support roles.
• Comfortable handling phone interactions and inquiries.
• Strong attention to detail and data accuracy.
• Ability to navigate multiple software platforms and CRMs.
• Proven ability to work independently in a remote environment.
• Clear and coherent English communication, written and verbal.

Responsibilities

• Provide clinical and administrative support to a US-based outpatient practice.
• Conduct outbound pre-visit calls and document patient information.
• Handle inbound inquiries and follow established protocols.
• Document CC, HPI, medications, and medical history in the EHR/EMR.
• Coordinate referrals, testing, and care with physicians and facilities.
• Prepare patient charts and ensure timely, HIPAA-compliant documentation.
• Support real-time clinical documentation and chart review as needed.
• Maintain professional, accurate patient records and follow workflows.
• Communicate with patients and providers to coordinate care and status updates.
• Escalate clinical questions to the appropriate staff and maintain patient confidentiality.
