Qualifications

• Proficiency with Electronic Health Record (EHR) systems, HIPAA-compliant communication tools, and office software.
• Exceptional verbal and written communication with empathetic listening.
• Outstanding time-management, attention to detail, and ability to manage multiple care tracks simultaneously.

Responsibilities

• Act as central hub of communication between patients, physicians, specialists, and care teams to ensure coordinated services.
• Assist in building, updating, and monitoring patient care plans, track progress, and schedule follow-ups.
• Maintain accurate EHR/EMR records, process referrals, verify insurance eligibility, and handle inquiries via phone/email/chat.
• Connect patients with community resources, home care services, equipment providers, and financial support programs.
• Conduct routine check-ins to monitor adherence, address barriers, and educate patients on care plans.
• Ensure all activities adhere to HIPAA regulations and SOPs.
• Identify delays or gaps in care and escalate to clinical leadership.
