Simply Source Outsourcing is seeking a highly organized Head of Intake & Scheduling Coordination to oversee patient referral intake, clinician scheduling, and care coordination for a growing US-based home healthcare staffing organization.

This remote, leadership role requires managing referrals and coordinating clinician assignments with partner agencies, using CliniConnects, and ensuring timely communication.

Qualifications

• Experience in US healthcare intake, patient scheduling, home health operations, or care coordination.
• Excellent written and verbal English communication skills.
• Strong organizational skills and ability to manage multiple referrals and scheduling requirements simultaneously.
• Knowledge of HIPAA regulations and patient information confidentiality.
• Strong attention to detail and accuracy in data entry and documentation.
• Ability to work independently, take ownership of tasks, and resolve operational issues proactively.
• Comfortable working in a fast-paced environment requiring prompt responses and consistent follow-through.
• Willing and available to work Monday to Friday during US Pacific Time business hours.

Responsibilities

• Process patient referrals from intake to scheduling coordination.
• Coordinate clinician assignments based on location, availability, and discipline.
• Track visits, SOCs, recertifications, and discharges; ensure timely communication with agencies.
• Develop SOPs for referral intake and scheduling; maintain platform workflows in CliniConnects.
• Provide daily referral status and scheduling updates to partner agencies and management.
