Job summary

Pathways Health Ltd is seeking an inspirational Team Coach (full-time) in Manukau, Auckland to lead two mobile teams—Counties Manukau Mobile and Intensive mobile. You will support tāngata whai ora on recovery journeys, supervising clinicians, peer workers, and care staff.

You will report to the Service and Relationship Manager and drive high-quality community-based care, data-informed decisions, and sustained partnerships across sectors.

Qualifications

  • Registered HPCA health practitioner with current APC
  • Knowledge of Te Tiriti o Waitangi and cultural safety
  • Leadership experience in health/social care teams
  • Experience in forensic and AoD settings preferred

Responsibilities

  • Oversee care of tāngata whai ora on mobile caseloads in Counties Manukau and Intensive mobile
  • Manage resources and drive service improvement incl. financial management
  • Lead and coach kaimahi; recruit, develop and manage performance
  • Develop partnerships across the sector and with external stakeholders
  • Support team to exceed personal best and grow services to meet community needs

The role | Mō tēnei tūranga mahi

We’re looking for an inspirational and motivated leader to takeup the exciting opportunity as full-time Team Coach (TC) located in Manukau,Auckland leading two mobile teams – Counties Manukau Mobile and Intensive mobile.

You’ll be reporting to the Service and Relationship Manager andleading a team of Support Workers/ Peer Support Workers. Together,you will support tāngata whai ora on their recovery and wellness journeyby helping them develop effective coping strategies, build life skills, engagewith their communities, and achieve their self-identified goals.

As the TC you’ll play a vital role in shaping the future of our service.Your responsibilities will include:

  • Oversee the care of tāngata whai ora on the Counties Manukau mobile and Intensive mobile caseloads to ensure they receive exceptional support in the community.
  • Support the efficient and effective use of resources for the teams while fostering innovation and improvement of service which includes financial management of the service.
  • Lead and support kaimahi, including regular individual coaching sessions, recruitment, professional development and performance management.
  • You will support the team to exceed their personal best in everything they do.
  • Develop and grow our services to meet emerging community needs.
  • Understand and use data that demonstrates our mahi in line with contractual obligations.
  • Grow strong and collaborative inter sector networks and partnerships.

This is a permanent, full-time position. You will be working Monday – Friday, 8:30am – 5pm. Based in Manukau. You will also contribute to anon-call roster.

About You | Mōu:

We are looking for a registered Clinician with some clinicaland leadership experience. You will have some understanding of te ao Māori and the wellbeing of tāngata whai ora andtheir whānau. You will bring a strong sense of purpose to your mahi, valuing whanaungatanga and leading with manaakitanga. In this role, you will nurture a safe, inclusiveenvironment, role model positive wellbeing, and empower kaimahi to grow and thrive as they support others on their journey.

  • HPCA regulated health practitioner, registered with acurrent APC (Registered Nurse, Social worker, OT, DAPANZ)
  • Knowledge of, anddemonstrable commitment to, Te Tiriti o Waitangi
  • Cultural awareness and acommitment to supporting Māori andPasifika whānau.
  • Ability to developcollaborative partnerships across the sector.
  • Relevant experience inleadership and managing others.
  • Forensic and AoD experience.
