Job summary

Pathways Health Ltd in Christchurch seeks a health professional with leadership experience to coach a team of health coaches and support workers delivering primary health care in GP practices.

The candidate will work within the Te Tumu Waiora programme, supporting trauma-informed care and building strong stakeholder relationships across primary health and NGO sectors. A current NZ registration and driver’s licence are required.

Qualifications

  • Health professional with current registration and frontline management experience in primary health care.
  • Ability to work within GP practices and with health coaches.
  • Knowledge of trauma-informed care and Te Tiriti o Waitangi would be advantageous.

Responsibilities

  • Provide frontline management to a team of health coaches and support workers across multiple GP practices.
  • Develop and maintain strong relationships with GP practices and partnering NGOs.
  • Contribute to a national rostered after-hours service on a rotating basis.

The opportunity |Te Whiwhinga mahi

Te Tumu Waiora is part of access and choice across greater Christchurch providing mental health support through medical practices in primary care settings with health coaches and support workers.


In your capacity as the Team Coach and part of the Pathways regional leadership team, you’ll provide frontline management and support to a team of health coaches and support workers across a number of medical practices in greater Christchurch. You’ll motivate your team and encourage a strong sense of inclusion within Pathways, especially as your team collaborates without daily interaction.


You’ll also be able to develop and maintain strong relationships with GP practices and frontline managers from other partnering NGOs.


Ideally you will have a health professional background, and experience working as a frontline manager in a primary health context.



  • Bepart of a leadership team, attend regular meetings and Team Coachprofessional development sessions

  • Contributeto a National rostered after-hours service on a rotating basis.


You | Mōu

The ideal candidate will be a health professional with a current registration who has experience working in the mental health industry, ideally with management and leadership knowledge. This could be a Nurse, Social Worker, Occupational Therapist or AOD with understanding of primary health sector an advantage, and the ability to build strong relationships with a number of stakeholders across primary health and NGO sector.


You will have;


  • Experiencein working with and an understanding of trauma-informed care.

  • TertiaryHealth qualification in Social Work, Occupational Therapy or Nursing andregistration with professional body.

  • Acurrent full New Zealand driver’slicence.

  • Acurrent New Zealand first aid certificate or be prepared to obtain one.

  • Ideally,you’ll have knowledge of the Canterbury Health and Social Services sectors

  • Knowledgeof, and demonstrable commitment to, the articles of Te Tiriti o Waitangi
