Job summary

Pathways Health Ltd is seeking a health professional with leadership experience to coach a team delivering primary healthcare to tangata whaiora via GP practices in greater Christchurch. You will provide frontline management to health coaches and support workers across several medical practices, fostering inclusion and collaboration within Pathways while aligning with national rosters after hours.

Qualifications

• Experience in trauma-informed care.
• Tertiary health qualification in Social Work, Occupational Therapy or Nursing and registration with professional body.
• Current NZ driver's licence.
• Current NZ first aid certificate or willingness to obtain one.
• Know-how of Canterbury Health and Social Services sectors.
• Knowledge of Te Tiriti o Waitangi.

Responsibilities

• Provide frontline management and support to a team of health coaches and support workers across multiple GP practices in greater Christchurch.
• Be part of the leadership team, attend meetings and participate in Team Coach development sessions.
• Contribute to a National rostered after-hours service on a rotating basis.
• Develop and maintain relationships with GP practices and frontline managers from partner NGOs.

The opportunity |Te Whiwhinga mahi

Te Tumu Waiora is part of access and choice across greater Christchurchproviding mental health support through medical practices in primary caresettings with health coaches and support workers.

In your capacity as the Team Coach and part of the Pathways regional leadershipteam, you’ll provide frontline management and support to a team of healthcoaches and support workers across a number of medical practices in greaterChristchurch. You’ll motivate your teamand encourage a strong sense of inclusion within Pathways, especially as yourteam collaborates without daily interaction.

You’ll also be able to develop and maintain strongrelationships with GP practices and frontline managers from other partneringNGOs.

Ideally you will have a health professional background, and experience workingas a frontline manager in a primary health context.

• Bepart of a leadership team, attend regular meetings and Team Coachprofessional development sessions
• Contributeto a National rostered after-hours service on a rotating basis.

This is a permanent full-time role, workingMonday-Friday, 8.30am – 5.00pm, in Christchurch.

You | Mōu

The ideal candidatewill be a health professional with a currentregistration who has experience working in the mental health industry, ideallywith management and leadership knowledge. This could be a Nurse, Social Worker,Occupational Therapist or AOD with understanding of primary health sector anadvantage, and the ability to build strong relationships with a number ofstakeholders across primary health and NGO sector.

You will have;

• Experiencein working with and an understanding of trauma-informed care.
• TertiaryHealth qualification in Social Work, Occupational Therapy or Nursing andregistration with professional body.
• Acurrent full New Zealand driver’slicence.
• Acurrent New Zealand first aid certificate or be prepared to obtain one.
• Ideally,you’ll have knowledge of the Canterbury Health and Social Services sectors
• Knowledgeof, and demonstrable commitment to, the articles of Te Tiriti o Waitangi
