Job summary

Placements24 is seeking a dedicated Social Support Facilitator to join our remote team, empowering clients in Queenstown and surrounding areas by connecting them to social services, resources, and personal development opportunities.

You will conduct remote needs assessments, create personalized support plans, provide remote counseling, and coordinate with support groups while maintaining confidential records.

Qualifications

• Bachelor's degree in Social Work, Psychology, Community Development, or a related field.
• Proven experience in social work, counseling, or community support roles.
• Strong understanding of social welfare systems and available community resources.
• Excellent remote communication, active listening, and empathy skills.
• Proficiency in using video conferencing platforms, CRM systems, and other digital tools for client management.
• Ability to work independently, manage time effectively, and maintain professional boundaries in a remote setting.
• Knowledge of issues affecting vulnerable populations in the Eastern Cape region is advantageous.

Responsibilities

• Conduct remote needs assessments to identify clients' social, emotional, and practical support requirements.
• Develop personalized support plans and guide clients in accessing relevant community resources and services.
• Provide remote counseling and emotional support to individuals facing various life challenges.
• Facilitate connections between clients and support groups, mentorship programs, and skill-building opportunities.
• Maintain detailed, confidential client records using secure online platforms.
• Liaise with family members and other stakeholders, with client consent, to enhance support systems.
• Monitor client progress and adapt support strategies as needed to ensure effective outcomes.
