Job summary

Placements24 seeks a compassionate Remote Social Support Facilitator to join the Community & Social Care team. This fully remote role provides emotional, practical, and informational support to clients via virtual channels, helping individuals access resources and improve well-being.

You will guide clients in identifying needs, plan actionable steps, and maintain confidential records. Strong virtual communication, empathy, and problem-solving are essential.

Qualifications

• Bachelor's degree in Social Work, Psychology, Sociology, or a related field.
• Proven experience in providing social support, counseling, or case management, preferably with remote capabilities.
• Proficiency with virtual communication tools (Zoom, Teams, etc.) and digital record-keeping.

Responsibilities

• Provide remote one-on-one support to clients via video conferencing, phone calls, and secure messaging platforms.
• Assist clients in identifying needs and developing actionable plans to achieve goals.
• Guide clients in accessing community resources, social services, and support groups.
• Offer empathetic listening and emotional support to clients facing various life challenges.
• Educate clients on available resources and empower them to utilize them effectively.
• Maintain accurate and confidential electronic records of client interactions and progress.
