Job Description:

Carry out social work tasks to a satisfactory professional standard in accordance with the policies of the Social Services Committee and the instructions of the Service Unit Head. Comply with and meet the standards set out by the policies and procedures jointly agreed between Social Services and Dorset Healthcare University NHS Foundation Trust.

Account to the Service Manager for the quality of work and provide information on the nature of the work.

Ensure that support plan reviews are carried out in a timely and person-centred way, evidencing the need for continuing support.

Undertake comprehensive assessments and contribute to risk assessments and risk management plans for adults.

Undertake care coordinator role for designated caseloads in accordance with Dorset Healthcare University Foundation Trust policies and procedures.

Undertake carers assessments and work with families.

Provide information and advice to adults and their carers to promote health, future planning, effective use of services, and personal responsibility, maximizing prevention and recovery services.

Encourage and support self-care, family, and community involvement, promoting choice and control, including the use of personal budgets and direct payments where possible.

Conduct best interest assessments, considering the Mental Capacity Act.

Assist adults in understanding the support and care available from the Council through effective communication and involvement, ensuring resource use aligns with person-centred outcomes.

Act as an Approved Mental Health Professional (or undertake training) and make hospital admissions under the Mental Health Act.

Serve as a Safeguarding Adult Practitioner (or undertake training).

Train as a Deprivation of Liberties Assessor.

Perform Care Manager duties as required, in accordance with statutory responsibilities and eligibility criteria.

Maintain essential records for statistical and operational purposes, ensuring compliance with quality standards and data protection.

Participate in team meetings, multi-agency risk meetings, child protection, safeguarding, and other relevant meetings.

Participate in out-of-office hours standby and statutory functions as authorized.

Engage in training and professional development.

Manage a caseload of a specialized nature, providing recovery-focused interventions.

Provide consultancy services within the team or service unit.

Undertake developmental roles within the team or unit.

Prepare for supervision and appraisal, demonstrating continuous professional development.

Prepare reports for court and give evidence in legal proceedings as required.

Undertake liaison roles, e.g., dual diagnosis link worker.

Perform other duties as required, consistent with the post level.

Comply with all policies, statutory requirements, and legislation, including the Equality Act, Health and Safety at Work Act, and Data Protection Act.

Maintain registration with SWE if required, and undertake statutory tasks related to the service responsibilities in accordance with legislation and the Council’s competence framework.
