Job description

It’s an exciting time for our Urgent Community Response (UCR) Service as we continue to build on our successful model. UCR services work with patients in their homes across Sussex communities to avoid hospital admissions and support those being discharged home.

Our multidisciplinary teams work flexibly to provide a 7‑day service, 365 days a year, between 8am and 8pm. They deliver:

  • A rapid response to people in the community who are experiencing acute episodes of ill-health.
  • Acute reablement for up to 10 days, supporting people to achieve functional goals and work towards independence with activities of daily living.

We are looking for Care Coordinators to join us, so if you’re an enthusiastic and motivated individual looking to be part of a team providing excellent cutting‑edge urgent community care, we’d love to hear from you!

Please note: Due to recent changes in UK immigration and visa rules, this role is unlikely to be eligible for Skilled Worker visa sponsorship. Applicants who do not currently have the right to work in the UK may not be considered. Candidates requiring sponsorship should review the eligibility criteria on thegov.uk website before applying to assess their eligibility for a Certificate of Sponsorship for this role.

In this role, you will be the point of contact for service users, their families, and carers when information on their crisis and short‑term care support is required. This will include enquiries about anticipated visits, providing updates, and listening to their concerns.

A key element of your duties will be processing referrals for reablement support and making onward referrals via the appropriate pathway for the patient, using criteria guidelines and seeking advice from relevant team members when required. You will sign‑post these referrals to MDT members and other services as appropriate while monitoring, tracking, and reporting on progress within the patient pathway. This will involve escalating any issues or delays to the Operational Team Lead for appropriate action.

We are the main provider of NHS community services across East and West Sussex, with 6,000 staff serving 1.3 million people. We deliver essential care to adults and children, helping them manage their health, avoid hospital admissions, and reduce hospital stays.

Our Trust vision is to provide excellent care at the heart of the community. We offer opportunities across medical, clinical, support, and corporate services.

Why work for us?
  • Positive 2024 NHS Staff Survey results, highlighting compassionate leadership and wellbeing
  • Varied environments: community hospitals, patients’ homes, and bases across Sussex
  • Flexible working options: part-time, flexi-time, annualised hours, and flexi-retirement
  • Excellent training, development, and research opportunities
  • Cost‑effective workplace nurseries in Crawley, Hove, and Brighton
  • Active EMBRACE, Disability & Wellbeing, LGBTQIA+, and Religion & Belief networks
  • Level 3 Disability Confident Leader and Veteran Aware Trust
  • Beautiful Sussex location near the South Downs and coast

Our values—Compassionate Care, Working Together, Achieving Ambitions, Delivering Excellence—guide everything we do.

We embrace diversity and encourage applications from all backgrounds, particularly from ethnically diverse, disabled, and LGBTQIA+ individuals. We aim to create an inclusive environment and support reasonable adjustments during recruitment.

This post may close early if sufficient applications are received.

You will need to be able to drive with a full UK licence and access to a vehicle due to the geographic area covered by the team.

You will be required to work from/travel between two sites, as required, as the team operates from both Lewes & Haywards Heath office bases.

Further information is available in the attached job description and person specification.

This advert closes on Sunday 28 Dec 2025.
