We are seeking a compassionate and motivated Health Care Support Worker to join our Learning Disability Acute Liaison Team. This is an exciting opportunity to support people with learning disabilities across acute hospital settings in Aneurin Bevan University Health Board.
You will play a vital role in reducing health inequalities and improving access to equitable, person-centred healthcare for patients with learning disabilities during their hospital journey.
This is a fixed term post until 13th June 2026 to cover the needs of the service
Provide face-to-face support to people with learning disabilities in inpatient and outpatient settings.
Help ensure patients' needs are communicated clearly to clinical staff.
Promote safe, equitable care by supporting behavioural strategies and health improvement plans.
Act as an advocate for individuals with learning disabilities and their families/carers.
Assist in preparation and delivery of training sessions to raise awareness of learning disability needs.
Engage in data collection and reporting as required to support service improvement and patient care planning.
Maintain a flexible, patient-centred approach, respecting individual needs, dignity, and preferences.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
