We are seeking a compassionate and motivated Health Care Support Worker to join our Learning Disability Acute Liaison Team. This is an exciting opportunity to support people with learning disabilities across acute hospital settings in Aneurin Bevan University Health Board.

You will play a vital role in reducing health inequalities and improving access to equitable, person-centred healthcare for patients with learning disabilities during their hospital journey.

This is a fixed term post until 13th June 2026 to cover the needs of the service

Provide face-to-face support to people with learning disabilities in inpatient and outpatient settings.

Help ensure patients' needs are communicated clearly to clinical staff.

Promote safe, equitable care by supporting behavioural strategies and health improvement plans.

Act as an advocate for individuals with learning disabilities and their families/carers.

Assist in preparation and delivery of training sessions to raise awareness of learning disability needs.

Engage in data collection and reporting as required to support service improvement and patient care planning.

Maintain a flexible, patient-centred approach, respecting individual needs, dignity, and preferences.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

• NVQ Level 3 in Health and Social Care or equivalent demonstrable knowledge and experience Good standard of general education including GCSEs (or equivalent) in English and Maths Understanding of learning disabilities and the health inequalities experienced by this population Knowledge of person-centred care and the importance of reasonable adjustments safeguarding, and professional boundaries
• Additional qualifications related to learning disability, healthcare, or communication (e.g., Makaton, Easy Read, LD awareness). Awareness of the Mental Capacity Act and its application Familiarity with accessible communication methods and tools Understanding of primary and secondary healthcare pathways and the role of liaison services Positive Behaviour Management (PBM) technique

• Experience of working with individuals with a learning disability or other vulnerable groups Experience providing care or support in a healthcare, social care, or community setting Experience working as part of a team
• Experience supporting individuals in acute or hospital environments Experience delivering or supporting training or health education sessions Experience collecting and inputting data for reports or audits

• Excellent communication and interpersonal skills, including the ability to adapt communication to individual needs Ability to work independently within defined boundaries Good organisational and time management skills Competent in using basic IT systems (e.g., email, data entry, Microsoft Excel/Word) Compassionate, empathetic, and non-judgemental Committed to promoting inclusion, equality, and the rights of people with a learning disability Flexible and adaptable approach to changing demands Willingness to learn and participate in further training and development
• Skills in preparing accessible resources (e.g., Easy Read materials) Ability to produce basic graphs or summaries from data Enthusiastic about service improvement and innovation Ability to advocate for service users within a multi-disciplinary setting Welsh Language Skills are desirable levels 1 to 5 in understanding, speaking, reading, and writing in Welsh
