A regional healthcare provider is looking for a Community Assistant Practitioner to join their King's Lynn Community Nursing Team. The role involves assisting in care delivery for patients with long term conditions while working collaboratively within a multidisciplinary team. Working Pattern:
The service operates between 08:00 and 18:00 hours, seven days a week, and you will be required to work shifts between these times, but we are willing to explore candidates who would like to consider flexible working.

To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.
