Overview

We have an exciting opportunity for a registered nurse to join the North Urgent Community Response Team on a 12 month secondment / fixed term contract, to support urgent community 2 hour response for patients in their own home, with skills in palliative / end of life care, wound care and catheter care and a strong desire to further develop your community nursing skills and experience.

Responsibilities
  • To work within the Urgent Community Response Team to prevent unnecessary admission to hospital.
  • To assess and prescribe care pathways for patients in a crisis situation with long term conditions, the frail & elderly, palliative and end of life care and/or rehabilitation needs, to achieve quality of life and independence where possible, with support from Senior colleagues within the wider Urgent Community Response Team.
  • To act as the patient’s advocate, facilitating choice and patient empowerment.
  • To provide evidence based clinical/therapeutic interventions based on "best practice".
  • To work within the Urgent Community Response Team to support early discharge from hospital.
  • To work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.
  • To triage referrals on a rotational basis for the Urgent Community Response Team.
Requirements / Qualifications
  • Excellent communication skills and the ability to demonstrate holistic assessment.
  • The ability to demonstrate initiative and a keen interest in working with this patient group.
  • You must hold a full and valid driving licence and have access to a vehicle.
Additional Information

You will be required to work over a 7 day working week including weekends and will be hard working, motivated.
