Overview

The UCR Service provides urgent support, seven-days a week, to help prevent unnecessary hospital admissions. This is achieved by providing a two-hour crisis response delivered by a multi-skilled team of professionals. Support is provided at home or where they usually reside for people who are concerned their health or mobility is declining and are therefore at risk of admission to hospital. Our teams work closely with our local ambulance service to pull category 3 & 4 patients off the ambulance stack, supporting patients in receiving a rapid response in the community by teams with the skills to support them to remain at home.

The UCR service also provide a general virtual ward service, supported by digital technology and remote monitoring. Our multidisciplinary teams ensure that patients receive hospital-level care at home safely and in familiar surroundings, helping speed up their recovery while freeing up hospital beds for patients that need them most. As well as home visits, digital remote monitoring is utilised.

We are the main provider of NHS community services across East and West Sussex, with 6,000 staff serving 1.3 million people.

Responsibilities

The postholder will be required to work autonomously as an Advanced Clinical Practitioner, using these skills to assess, investigate, diagnose, treat and manage patients who may have co‑existing and diverse factors making management complex. They will develop, utilise and maintain skills beyond the scope of normal practice (for example advanced physical assessment, diagnostics and prescribing). Within the role there will be a requirement to plan and direct the management of patients including accessing services within the multi‑disciplinary team and wider health, social care, education, vocational and voluntary sector services as appropriate to the role. The postholder will lead on the strategic development of Trust‑wide services in their specialist area, working with other strategic and clinical leads as appropriate. They will also lead and participate in education programmes designed to support other clinical staff skills within their speciality.

The postholder will provide peer support to clinical staff working with patients from the direct caseload or those involved in the management of patients. They will assist in the planning, co‑ordinating and development of any newly implemented service plans as well as being responsible for ensuring that relevant legal and regulatory frameworks are acknowledged through local governance procedures.

The postholder will also be competent at establishing protocols and guidelines for the growth of the service and to undertake and support research in the future development of their speciality.

Qualifications & Requirements

You will need to be able to drive with a full UK licence and access to a vehicle due to the geographic area covered by the team.
