Job summary

Cheshire West and Chester Council is seeking a Therapy Assistant Practitioner to join our Promoting Independence Team on a 24-hour per week basis. You will work with Occupational Therapists to deliver person-centred interventions that promote wellbeing, independence, and safer daily living at home.

Carry out strengths-based assessments, support equipment provision and TEC solutions, and help individuals achieve meaningful goals in their communities.

Qualifications

  • Enthusiastic, motivated and compassionate Therapy Assistant Practitioner.
  • Ability to work collaboratively with Occupational Therapists to deliver person-centred care.
  • Strong commitment to promoting independence and wellbeing.

Responsibilities

  • Carry out strengths-based assessments to identify individuals' needs, goals, risks, strengths, and priorities, ensuring timely and proportionate interventions that promote independence.
  • Support and empower individuals to maximise quality of life, achieve personal outcomes, and develop skills to remain independent at home and in communities.
  • Work with individuals and carers to identify what matters most and address barriers to independence.
  • Liaise with health, social care, voluntary, and third-sector partners to coordinate person-centred interventions and joint assessments.
  • Undertake functional assessments and apply strengths-based practice to identify practical solutions for goals and independence.
  • Assess, recommend, prescribe, and review equipment, minor/major adaptations, and TEC solutions to address social care needs.
  • Provide specialist guidance on equipment, adaptations, housing solutions, and reablement approaches.
  • Maintain accurate electronic records, reports and case documentation for service delivery and development.
  • Contribute to the development and continuous improvement of the Promoting Independence Team.
