Job summary

Dorset HealthCare is seeking a Therapy Assistant Practitioner to join our Purbeck ICRT team in Wareham, with a rotational post between community and Swanage Community Hospital. The role involves working with adults in their homes and care homes to promote independence, requiring a car for community visits and a 37.5 hour week.

Qualifications

• Must have a driving licence and car to visit patients in the community.
• Right to work in the UK and no visa sponsorship available.
• Rotational post between community and Swanage Community Hospital (37.5 hours/week).
• Opportunities for learning and Apprenticeships in Nursing, Physiotherapy, and Occupational Therapy.

Responsibilities

• Carry out holistic assessments and formulate treatment plans.
• Monitor progress and record outcomes against goals.
• Support patients to regain independence and return home.
• Deliver rehab interventions, under indirect supervision of a registered therapist.
• Manage a clinical caseload and visit patients in the community.
