Job summary

Wye Valley NHS Trust in Herefordshire is seeking a Band 4 Therapy Assistant Practitioner to join our Community Team. You will work with Physiotherapists, Occupational Therapists and other staff to assess and deliver rehabilitation in patients' homes and bedded units, supporting discharge pathways.

You will assess independence, provide equipment and rehabilitative techniques, and contribute to care planning and service development while maintaining high professional standards in a busy community

Qualifications

• Foundation degree / degree in health & social care or Level 5 apprenticeship (e.g. Assistant Practitioner) or equivalent clinical experience.
• Functional Skills Level 2 or Maths & English GCSE (Grades 9-4 or A*-C).
• Care Certificate.

Responsibilities

• Provide direct high quality therapeutic care and support to patients in the community.
• Independently plan and deliver individual or team tasks within scope of practice.
• Contribute to service development and 7-day working requirements.

Education
