Job summary

Derbyshire County Council is hiring a part-time Social Care Practitioner (18.5 hours) for the Amber Valley Occupational Therapy team. The role covers the South of the County with a focus on enabling adults to maximise independence through strength-based, holistic assessments.

You will work within a supportive team, with supervision from OT managers and Senior Practitioners, and will have access to flexible/hybrid working, ongoing training, and generous annual leave.

Qualifications

• Person-centred, holistic, strengths-based approach.
• Assess activities of daily living.
• Knowledge of physical, learning disabilities and/or mental health conditions.
• Knowledge of equipment and adaptations.
• Ability to manage and prioritise casework.
• Good communication and ability to form positive relationships with partners.

Responsibilities

• Deliver person-centred, strengths-based occupational therapy assessments.
• Assess activities of daily living and determine support needs.
• Travel to service users for home visits.
• Collaborate with carers, families and professionals to coordinate care.
• Maintain accurate records and case notes.
