Job summary

Lincolnshire Community Health Services NHS Trust seeks a Promoting Independence Therapy Assistant in Gainsborough to support post-discharge assessments and home-based reablement under supervision of registered therapists.

You will work within a multi-disciplinary team to promote independence, conduct environmental home assessments, and help plan simple patient interventions while ensuring equipment safety and appropriate referrals as required.

Responsibilities

• Assist in assessment and rehabilitation of individuals post discharge with support from registered therapists.
• Provide reablement care to patients in their own homes.
• Work with the multi-disciplinary team to promote independence and prevent dependence.
• Undertake environmental assessment within the patient’s home, often single-handed and without direct supervision.
• Communicate effectively with patients, carers, health professionals and external agencies.
• Collaborate with other agencies to determine long-term care needs.
• Participate in delivery of complex care as prescribed by the qualified practitioner.
• Plan and progress simple patient interventions with autonomous assessment and planning under indirect supervision.
• Be responsible for safe use of equipment within your scope of practice.
