Job summary

Gloucestershire Health and Care NHS Foundation Trust is launching a countywide Home First service and seeks enthusiastic Therapy Assistants to join the team. You will work alongside Occupational Therapists and Physiotherapists, delivering person-centred therapy in people’s homes after hospital discharge and managing a delegated caseload.

You will assess needs, provide rehabilitation, and promote independence while coordinating with a multidisciplinary team to ensure safe, effective discharge

Qualifications

• Level 3/4 qualification in Health & Social Care or related healthcare subject.
• GCSE English and Maths at grade 4/C or above.
• Care Certificate.

Responsibilities

• Support people in their own homes following hospital discharge to maximise independence and wellbeing.
• Deliver person-centred therapy and reablement interventions within competency framework.
• Undertake holistic assessments and monitor progress to adapt interventions.
• Provide advice and education to individuals, families and carers; maintain accurate clinical records.
