ABOUT THE ROLE

Shift details 19:45 – 08:00, four nights per week on week 1 and three nights on week 2, alternating weekends.

Your focus as Team Leader will be to support the home management team in delivering high-quality care with a difference by providing a safe, caring, innovative, open, supportive, and inclusive environment for residents, visitors, and staff.

  • Lead the person‑centred care planning process by implementing, monitoring, and undertaking monthly evaluations, completing care plan audits, developing action plans, and being responsible for completing the action plans.
  • Assist in the smooth transition of any discharges from the home, including documentation, medication, and keeping the residents' next of kin fully informed where appropriate.
  • Monitor the well‑being of each resident, be aware of any change impacting care and care planning, and ensure that changing needs are met and written records are amended to reflect any variation.
  • Practice safe systems of work across a range of tasks and the moving and handling of residents by assessing risks and having due regard for personal safety and the safety of residents, visitors, and staff.
