Rolling Rota and every other weekend off
12-hour shift's 8am till 8pm locations are across Barnsley area
Rate of pay £13.21 per hour
Purpose of Position
To work as part of a team to ensure effective support of an individual with learning disabilities, Autism or Mental Health needs.
To provide day to day support to enable individuals to maintain their own tenancy within the community in all aspects of daily living, life planning and community participation.
To support people with learning disabilities, Autism or Mental Health needs. to experience life opportunities which promote the growth of the individuals to their maximum potential.
Key Tasks and Responsibilities
- Be aware of the specific needs of individuals with learning disabilities, Autism or Mental Health needs. their individual support plans and actions required to achieve positive and meaningful outcomes.
- To report immediately any incidence, suspicion or sign of abuse of an individual.Or any other matter pertaining to safeguarding of a Vulnerable Adult.
- To support individuals in maintaining their tenancy and liasing with their landlord
- Assist and support individuals who need help with their personal needs such as dressing, undressing, bathing and using the toilet.
- Assist with individuals who have mobility problems and other physical disabilities
- Help in the promotion of mental and physical activity of the individual, through communicating effectively with them and in the promotion of constructive use of leisure time.
- To actively assist the individual in order to achieve their personal goals.
- Assist and support individuals within their own home with tasks such as, cleaning, cooking, and laundry.
- Assist and support individuals with budget planning, paying bills, shopping
- Assist and support individuals in maintaining a safe and secure home
- Act as co-keyworker to individuals and in the upkeep of their records and support plans.
- Contribute to the reviewing of individual support plans and preparation of reports and attend reviews as appropriate.
- To be aware and keep up to date with the contents of everyone’s person centered support plan.Following all guidelines and risk assessments in place.
- To develop positive working relationships with individuals and act as a Mentor
- Flexibility to carry out waking night duties / sleep ins if required
- Maintain confidentiality in line with policies and procedures on all matters relating to individuals and Sun Healthcare.
- Liaise with individuals’ families, appropriate professional staff and the local community in furthering the development and interests of the individual.
- Assist with medication administration as necessary.
- To undertake all duties in line with all Sun Healthcare Policies and Procedures.
- To ensure, as far as reasonably practicable, observance and adherence to the Health and Safety at Work Act.To take all necessary steps to ensure the personal safety of individuals you support, other staff and self.
- To be aware of relevant legislation such as Care Quality Commission, Data Protection and comply with all regulations.
- To be aware of requirements of Supporting People and Quality Assurance Framework Standards.
- To ensure that all relevant E-learning training remains as a minimum requirement at 90%. Also, completion of the Care Certificate within 12 weeks of commencement of role (unless an equivalent or higher qualification in a relevant field has been completed previously). Also attend identified training courses to further knowledge and understanding of learning disabilities / complex needs and all aspects of job role.
- Participate in supervision sessions and annual appraisal
- Carry out any other duties that may be reasonably expected of a Support Worker at the discretion of the Registered Manager.
