Job summary

This is an excellent opportunity for a candidate keen to pursue a career in geriatric and general medicine. The post will initially be based with the Orthogeriatrics team on Emmer Green Ward (the hospital's 24-bed Hip Fracture Unit), but with scope to rotate through other elderly care wards during the 6 months. There will be a commitment to the medicine on-call rota, including hospital at night shifts. There is an opportunity to review patients in the Rapid Access Clinic for the Older Person (RACOP), reviews of older and frail patients under the care of other specialties within the hospital, as well as contribute to the peri-admission reviews of hip fracture patients. The Emmer Green team works under the supervision of consultants with a special interest in orthogeriatric medicine, who rotate every 8 weeks. Typically, there are 2 consultants allocated to the ward at any time, along with a specialty doctor, IMT trainee, FY2, up to 3 FY1s, and a Physician's Assistant.

Main duties of the job
  • Attend Consultant and own ward rounds
  • Work alongside the Consultant to provide expertise to the frail older patients under the care of the hip fracture unit
  • Gather collateral history from patients and their families or GP
  • Work alongside the multidisciplinary team to help patients to achieve their rehabilitation goals
  • Attend and contribute to the 'best interest meetings' with families of patients with cognitive impairment to plan for future care
  • Review Elderly Care ward referrals from other specialties as per rota
  • Attend RACOP (Rapid Access Clinic for the Older Person) as per rota
  • Undertake responsibilities from the medicine on-call rota, including hospital at night responsibilities

Job description – responsibilities

The appointee will have an overriding duty of care to patients and are expected to comply fully with best practice standards. The appointee will have a responsibility to comply with Trust policies for personal and patient safety and for prevention of healthcare-associated infection (HCAI); this includes a requirement for rigorous and consistent compliance with Trust policies for hand hygiene, use of personal protective equipment and safe disposal of sharps. Knowledge, skills and behaviour in the workplace should reflect this; at annual appraisal you will be asked about application of practice measures known to be effective in reducing HCAI.

Person Specification – Professional Qualifications

Clinical Experience & Skills
  • Experience in Geriatric Medicine
  • Evidence of achievement of CT2/IMT2/3 competencies by the start date of the post; or able to demonstrate achievement of the necessary equivalent competencies

  • Minimum 1 year NHS experience
Clinical Knowledge
  • Evidence of experience in acute medical specialties, with experience of managing patients on unselected medical take

  • Specific management of geriatric syndromes including dementia
Audit Management & IT
  • Ability to work within clinical governance guidelines.
  • Undertake audits and present data as required.
  • Good IT skills, use of patient and hospital database.

  • Completion of Quality Improvement Project
