A leading healthcare provider in the UK is seeking a dedicated Respiratory Practitioner to enhance patient care within the community. The successful candidate will provide physiotherapy and support for respiratory patients, managing transitions of care and engaging with hard-to-reach populations. The Integrated Cardio-Respiratory Services is a combined service working across hospital and community pathways, providing community services in clinic and home-based settings. The service is a consultant led multidisciplinary team providing a range of service including; diagnostic clinics, COPD and asthma disease optimisation, exacerbation management, hospital discharge, home oxygen, pulmonary rehabilitation, dyspnoea management and primary care education and support. The aim is to provide a more accessible, efficient, cost-effective service integrating respiratory services across secondary and primary care, to enable continuity of care and give respiratory patients the opportunities to have access to care closer to their homes and end of life care in their homes. The successful post holder will work closely with the MDT and dedicated social prescriber to improve access to PR and alternative models of therapy for patients who cannot attend PR. They will lead on home rehabilitation and community engagement initiatives with a dedicated focus on improving accessibility within the hard to reach populations. They will have the opportunity to develop quality improvement and project management skills, working with a team of expert clinicians to promote the service through poster presentations, abstract submission and conference attendance.
