A leading healthcare provider in Greater London is recruiting a Specialist Respiratory Practitioner to integrate hospital and community respiratory services. The role involves delivering quality care to adult respiratory patients, leading home rehabilitation initiatives, and collaborating with a multidisciplinary team. Significant experience in respiratory care, a relevant degree, and registration with HCPC or NMC are required. We are recruiting for a Specialist Respiratory Practitioner within the Integrated Cardio-Respiratory Services. This is a combined service working across hospital and community pathways, providing community services in clinic and home-based settings.
The service is a consultant led multidisciplinary team providing a range of service including; diagnostic clinics, COPD and asthma disease optimisation, exacerbation management, hospital discharge, home oxygen, pulmonary rehabilitation and dyspnoea management.
The aim is to provide a more accessible, efficient, cost-effective service integrating respiratory services across secondary and primary care, to enable continuity of care and give respiratory patients the opportunities to have access to care closer to their homes and end of life care in their homes.
The successful post holder will work closely with the multi-disciplinary team and dedicated social prescriber to improve access to pulmonary rehabilitation and alternative models of therapy for patients who cannot attend pulmonary rehabilitation.
They will lead on home rehabilitation and community engagement initiatives with a dedicated focus on improving accessibility within the hard to reach populations.
They will have the opportunity to develop quality improvement and project management skills, working with a team of expert clinicians to promote the service through poster presentations, abstract submission and conference attendance.
