Overview

Are you an experienced Social Worker with a strong background in mental health, Section 117 aftercare, and exposure to Continuing Healthcare (CHC) assessments? We're working with a forward-thinking local authority looking for an interim professional to join their Adult Social Care team on a fully remote basis.

Responsibilities
  • Undertake assessments and reviews under the Care Act 2014, with a focus on individuals eligible under Section 117 Mental Health Act.
  • Collaborate with multi-disciplinary teams to contribute to CHC assessments, ensuring robust and accurate social care input.
  • Support discharge planning and aftercare arrangements for individuals leaving hospital under the Mental Health Act.
  • Maintain accurate, person-centred case records and documentation using electronic case management systems.
  • Ensure compliance with statutory duties and safeguarding responsibilities.
Requirements
  • Qualified Social Worker with current Social Work England registration.
  • Proven experience working within mental health services, ideally within a local authority or NHS context.
  • Strong working knowledge of Section 117, Care Act, and experience contributing to CHC assessments.
  • Confident working remotely and independently, with access to a secure and suitable home working environment.
  • Excellent communication and partnership working skills.
