Responsibilities
  • Undertake Care Act Assessments, Mental Capacity assessments, and facilitate Best Interest outcomes to ensure client needs are met.
  • Perform Community Deprivation of Liberty assessments and manage CHC checklists and Decision Support Tools (DSTs) effectively.
  • Operate from a Strengths-Based perspective, demonstrating Least Restrictive Practice to empower service users.
  • Support the team duty function by triaging referrals and effectively prioritising case management activities.
  • Collaborate confidently with professional partners in a Multi-Disciplinary Team (MDT) approach to achieve holistic care.
