Key Responsibilities
  1. Carry out comprehensive assessments and create tailored care plans for individuals with learning disabilities.

  3. Collaborate with families, carers, and other professionals to ensure a holistic approach to care.
  4. Monitor and review care plans regularly, adapting them to meet clients’ evolving requirements.
Person Specification
  1. Qualified and registered Social Worker with relevant experience in learning disabilities.
