Work Days/Hours: 20.5 hoursMonday 09.00-16.00, Tuesday 12.00-20.00 &Wednesday 09.00-16.00
Socialprescribing is a non-clinical role that empowers people to take control oftheir health and wellbeing by connecting them with appropriate community-basedservices and activities. As a Social Prescriber, you will supportindividuals, particularly those with long-term conditions, mental health issues,social isolation, or complex social needs by helping them access supportnetworks that improve their quality of life and reduce reliance on health andsocial care services. This role requiresthe social prescriber to work efficiently within the fast paced and ever-changingprimary care environment, delivering high quality support in a responsive andeffective manner.
Bournemouth East Collaborative Primary Care Network PCN,comprises four like-minded practices working together in East Bournemouth,Dorset, with a strong reputation on quality improvement and investing in itsemployees. The PCN serves a population of approximately 53,000 patients with adiverse demographic.

Enhanced Care Team for frail housebound patients and thosein care homes. The team includes visiting GPs, ANPs, Frailty Nurses, NurseAssociates, Healthcare Assistants, Care Coordinators and a Pharmacist.
