An exciting opportunity has arisen for a highly motivated Senior Practitioner to join our Early Intervention & Prevention Hub. The team has been reconfigured and developed following our recent review of social work.
You will be part of the Early Intervention and Prevention Hub, delivering social work services to adults who have experienced a change in their circumstances that has affected their usual abilities to live independently.
The team provides a short‑term social work response, which focuses on outcomes for individuals, maximising individual potential and minimising risk to independence, with an emphasis on choice and control.
Using a strengths‑based approach you will facilitate the provision of information, advice and assistance and access to universal services. You will ensure equity of access to tailored short‑term enabling care and support for adults affected by disability and ageing. This service focuses on ‘What matters’ to individuals and carers, maximising individual potential and minimising risk to independence, with an emphasis on voice and control.
The ability to greet customers through the medium of Welsh is a requirement for this post.
Protecting children, young people or adults at risk is a core responsibility of all council employees.
An Enhanced with Adults Barred list criminal records check by the Disclosure & Barring Service (DBS) is a requirement for this post.
