Exciting opportunity for an experiencedSenior Practice Nurse to join our friendly East London general practice. Youwill join a team of Practice Nurses and Healthcare Assistants and willundertake all aspects of the practice nurse role including cervical screening,baby immunisations and chronic disease management.

This roleprovides strategic and professional leadership and continuous improvementwithin the practice.

You will be joining a team ofthree nurses, two health care assistants and one assistant practitioner inaddition to the wider team which comprises GPs, Pharmacists and Physician Associate.The practice is a teaching practice, and we provide patient services to arichly varied, multicultural population which includes families, students andprofessionals.

Experience in General Practiceas a Senior Practice Nurse is essential. We encourage informal visits to ourHealth Centre.

Main duties of the job
  • The senior practice nurse willbe responsible for a number of clinical areas as well as leading in thereviewing of clinical policies and procedures.
  • Lead on clinical governance,regulatory compliance (including CQC and IPC), and patient safety systems.
  • Collaborate with partners,practice management, and external stakeholders to ensure best practice andpatient outcomes.
  • Managing governance, risk, andcompliance systems across the practice.
  • Providing strategic andprofessional leadership to the nursing team, ensuring high standards of patientcare, clinical governance, and alignment with the practices objectives.

Please note that additional admin time will be provided to support withthe management duties and responsibilities.

About us

Our Health Centre waspurpose-built in 1987 and is owned and maintained by the Partners. It has 21clinical rooms, plus a large educational room and undergraduate training room,and an administrative wing which provides office space for our administrators,secretaries, and managers.

The practice has a shared beliefin the delivery of high quality, patient-centred, and evidence-based healthcare. Patients interests are kept to the fore and there is a strong emphasison continuity of care and a commitment to continue developing patient services.Every patient and their family are allocated to a named GP upon registration.They are encouraged to see that GP for their routine care, though we understandthat on occasions a patient may wish to see a different doctor about specificproblems.

We are part of the Poplar &Limehouse Health & Wellbeing Network, along with 3 other local practices,which formed in 2006 to work collectively and improve health care provision inareas such as heart disease and diabetic care.

Partners have held externalroles locally that complement the clinical work such as lecturer posts at QueenMary University of London and leadership roles at the CCG and the Local MedicalCommittee.

We are also an Approved TrainingPractice for local GP training, for medical students from Barts & The RoyalLondon.

Job responsibilities
  • The senior practice nurse willbe responsible for a number of clinical areas such as health promotion, chronicdisease management, health prevention, as well as leading in the reviewing ofclinical policies and procedures.
  • Initiating and maintaininghealth screening and promotion clinics for identified areas of the practicepopulation, recognising where modification of systems or referral is necessary.
  • Managing the nursing care ofpatients with chronic disease, employing personal recall systems suitable tothe practice needs. This includes using the practice computer systems for inputof information for recall and screening purposes.
  • Providing specialist care forregistered patients in the following key areas: Asthma, Hypertension, Diabetes,COPD, Cervical Smears, Contraception, Baby immunisations.
  • Providing routine treatment ingeneral clinics, including Wound Management, Blood tests, and ECGs.
  • Lead on clinical governance,regulatory compliance (including CQC and IPC), and patient safety system.
  • Collaborate with partners,practice management, and external stakeholders to ensure best practice andpatient outcomes.
  • Managing governance, risk, andcompliance systems across the practice.
  • Providing strategic andprofessional leadership to the nursing team, ensuring high standards of patientcare, clinical governance, and alignment with the practices objectives.
Person Specification
Qualifications
  • UK Registered nurse with NMC;
  • Minimum 2 years post registration;
  • Experience working in General Practice setting as a Senior Practice Nurse;
  • Demonstrate an understanding of audit;
  • IPC;
  • Cervical Cytology;
  • Child and Adult immunisations;
  • Care of Long Term Conditions (LTCs);
  • Experienced in Wound care.
  • Trained in Anti-coagulation.
Knowledge and Skills
  • Excellent communication skills (written and oral)
  • Clear, polite telephone manner;
