Job description

The post holder will act as an experienced physiotherapist, helping to develop integrated health care teams in the community with a Multi-Disciplinary Team (MDT). The post holder will provide high quality physiotherapy interventions to a range of patients from different community settings such as frailty, rapid response / admission avoidance, intermediate care, falls prevention through to preventative action and support for self‑management of long term conditions. The caseload will include advanced assessment and treatment of patients, provision of individual and group education and treatment, provision of assistive devices (e.g. orthotics) and equipment as well as liaising with other services and agencies.

Main duties and responsibilities
  • To manage a defined caseload using evidence based /client centred principles to assess, plan, implement and evaluate interventions, maintaining associated records.
  • To provide a high standard physiotherapy service to the specialist caseload across various sites within Tower Hamlet as the service requires.
  • To perform advanced physiotherapeutic assessment of patients with diverse presentations and complex physical and psychological conditions to provide a diagnosis and develop and deliver an individualized treatment program.
  • To provide leadership for more junior staff, as agreed with senior staff.
  • To deputise for the Senior Physiotherapists in their absence as required.
  • To supervise Physiotherapy students on clinical placements.
  • To participate in CPD and develop skills and knowledge through participation in the in‑service training scheme.
  • To undertake lone working when required.
  • To be able to work flexibly and participate in potential future 7‑day working developments (when required).

Clinical Responsibilities
  • To be professionally and legally accountable for all aspects of own work, including the management of patients in your care.
  • To undertake all aspects of clinical duties as an autonomous practitioner.
  • To be responsible for maintaining accurate and comprehensive patient treatment records in line with legal requirements and Health & Care Professions Council (HCPC) standards of practice.
  • To understand the legal framework around gaining informed consent for treatment and to implement appropriate procedures in line with trust policies.
  • To have a comprehensive understanding of patients’ clinical presentations based upon advanced clinical knowledge.
  • To develop specialist knowledge of particular conditions and patient presentations specific to different diagnoses and treatments.
  • To undertake a comprehensive subjective and objective assessment of patients with diverse or complex presentation or multiple pathologies and use advanced clinical reasoning skills to accurately identify impairments, abilities and participation.
  • To develop timely and comprehensive discharge plans, considering appropriate future intervention in accordance with prognosis.
  • To assess clinical risk within own patient caseload and to be aware of risks involved in the caseloads of other team members.
  • To formulate, plan, deliver, evaluate and progress a patient‑centred physiotherapy treatment and management program by effective and efficient clinical reasoning, using current evidence based practice (e.g. Best Practice Statements, journal review, in‑service training, post‑graduate training, peer review).
  • To use valid and reliable outcome measures to evaluate patient progress, reassessing and adjusting treatment programs as indicated.
  • To assess and treat appropriate patients on a weekend day. This will include patients needing not only treatment for respiratory issues but also patients who require rehabilitation and long‑term management.
  • To work closely with the multi‑disciplinary team, acknowledging areas of uni‑ and inter‑professional expertise, ensuring optimal outcome for both patients and service.
  • To be an active multi‑disciplinary team member, ensuring a coordinated patient‑centred approach to service delivery. This will include multi‑disciplinary team discussion of patient care, prognosis, goal attainment and timely discharge planning.
  • To liaise with, and provide advice and education to, medical, nursing staff, social services staff and other AHPs regarding the 24‑hour management of patients on a daily basis and in all emergency duty situations.
  • To identify change in patient presentation and arrange for urgent or non‑urgent review by other disciplines.
  • To be responsible for safe and competent use of all equipment, appliances and aids by both patients and staff under your supervision.
  • To develop specialist knowledge of particular conditions and patient presentations as encountered through working in the community health teams.
Professional Responsibilities
  • To adhere to HCPC/ CSP Codes of Conduct and Standards of Practice at all times, ensuring that those working under your supervision do the same.
  • To positively promote the role of Physiotherapy.
  • To work within national and Trust (East London Foundation Trust) clinical guidelines, policies and standards and CSP guidelines. Monitor own and others quality of practice, ensuring safe working practices with reference to these. Have a basic understanding of the Vulnerable Adults policy and Child Protection Act.
  • To consistently maintain best practice, Continuing Professional Development and Career Pathway through, for example, attendance at postgraduate courses, in‑service training, journal club and Special Interest Groups, encouraging staff under your supervision to do likewise.
  • Maintain an up‑to‑date diary reflecting CPD activities and personal and professional development. To have agreed protected time in which to complete this.
  • To take responsibility for participating in PDP and meeting set objectives.
  • To participate actively in in‑service training programs, tutorials, Clinical Governance days, peer review and courses, both internal and external.
  • To participate in, and/or lead departmental and multi‑disciplinary audit and research initiatives into specific areas of clinical practice utilizing support and supervision from senior staff as required.
  • To undertake evidence‑based audit and/or research projects to further own and team’s clinical practice within each rotation.
  • To present findings to the therapy and wider team and be involved in resultant service development and changes to practice.
  • To provide clinical support and supervision to assistants and qualified physiotherapists working at a lower grade on a daily basis.
  • To attend necessary clinical educator and appraisal training.
  • To lead and/or contribute to the performance assessment and appraisal of qualified physiotherapists working at a lower grade and physiotherapy assistant staff on a daily basis, including taking the role of Appraiser at PDP meetings.
  • To supervise, educate and assess the performance of physiotherapy students to a graduate standard on a regular basis. To work in conjunction with universities to ensure the standard of practice and teaching meets the standards set by the degree level qualification.
  • To be able to provide formal teaching/training to other multi‑disciplinary team members if requested, and to teach basic skills/knowledge on an informal basis throughout day‑to‑day practice.
  • To provide learning/observational opportunities to a variety of medically linked students e.g. Physios’, Occupational Therapists, Medics, and Nurses, on an individually agreed basis.
Organisational
  • To be responsible for organising and planning own case‑load to meet service and patient priorities, allowing adequate time for non‑clinical duties, such as report writing, leasing within the team, across services and other agencies, and student supervision.
  • To deputise for the Band 7 in their absence on an agreed, short‑term basis, taking responsibility for operational management of the team (juniors, assistants, students and any A & C staff), allocating and organising the workload to meet service priorities.
  • To prioritise the team workload as temporary staffing shortages arise, e.g. short term sick leave.
  • To be flexible during the working day, allowing response to unexpected situations, e.g. acute patient illness, urgent referrals.
  • To assist in the induction of more junior staff members and students.
  • To be responsible for the supervision and co‑ordination of junior staff, students and assistants on a daily basis.
  • To be actively involved in the instigation, organisation and running of both in‑service training and external courses.
  • To be involved in the care and maintenance of equipment as per manufacturers guidelines and relevant policies, including the identification and reporting of faults and problems.
General
  • The CHTs if required by the population of Tower Hamlets, work over extended days (8am – 10pm) including weekends and the post holder would need to be able to work occasional shifts within these times as required by the CHTs.
  • To have responsibility for the health, safety and welfare of self and those you supervise and to comply at all times with the requirements of health and safety regulations.
  • To represent the profession and the Trust in a positive and professional manner at all times, contributing personally and professionally to the development of Physiotherapy and Barts Health to the best of your ability.
  • To attend and lead senior staff meetings.
  • To ensure confidentiality at all times, only releasing confidential information obtained during the course of employment to those acting in an official capacity in accordance with Trust policies and Caldicott Guidelines.
  • To adhere to the Trust’s incident‑reporting policies and procedures at all times, co‑operating with investigations undertaken in respect of this.
  • To be able to identify the need for, and participate in, flexible working.
  • To comply with all Trust policies including equal opportunities and manual handling.
  • To comply with the requirements of the NHS and Social Care related legislation including the Data protection Act 1994.
  • To carry out such duties as may be required by the professional/line manager consistent with the responsibilities of the grade.
  • To attend mandatory training and induction courses as designated by the Trust.
  • To abide by HCPC rules of professional conduct and local professional and quality standards.
