A leading healthcare provider in Birmingham is seeking a motivated Senior Nurse Assessor to support children's complex health needs. This community-based role involves case management, assessments, and collaboration with various services. Excellent clinical skills, knowledge of safeguarding, and a background in multi-agency working are essential. Continuing Care is the name given to a package of care that is arranged and funded by the NHS for children and young people who have an unmet health need that cannot be met by universal and specialist services.
We are currently looking for a motivated and enthusiastic children's nurse with case‑management experience who has excellent clinical knowledge as well as communication and assessment skills. The applicant should be resilient and able to work autonomously but also work as part of a team. Good knowledge of safeguarding, learning disability and mental health conditions and multi‑agency working is required.
An understanding of the continuing care framework is required alongside experience of case‑management and multi‑agency working as an essential requirement for the post, as close collaboration takes place with community and acute services as well as education and social care.
This is a community‑based post based in Birmingham and Solihull. The post holder will be expected to travel covering the whole area, with some out‑of‑area travelling required. We are currently working from home to support children and families.
The role is varied with the practitioner undertaking assessments, attending MDTs and delivering training to multi‑agency professionals.

The successful applicant will also need to work across the locality in collaboration with other team members and colleagues in other organisations and ICB Departments to ensure quality services are delivered to patients.
