• Registered Nurse with NMC registration.
• Experience in General Practice or community nursing preferred.
• Experience of managing Long Term Conditions.

Responsibilities

• Provide general nursing services including chronic disease management and immunisations.
• Work with GPs and nurse practitioners to provide holistic patient care.
• Manage QOF and long‑term condition targets.
