TheSt James Adult Medicine Team are looking to recruit a motivated rotationalspecialist clinical pharmacist. We have a host of different rotations for youto learn from including Older peoples Care, Respiratory and CF, Generalmedicine, Acute medicine, ED, SDEC.

Main duties of the job

Toprovide pharmaceutical care to a group of patients within a clinical area. Toprovide cover for other pharmacists within the relevant Clinical PharmacyTeam as appropriate. To support the work of the Lead Pharmacist for the areaby assisting in the production of drug-related policies, procedures andfinancial reports, and by performing horizon scanning activities.

Job responsibilities

Provide specialist clinicalpharmacy service to patients and the multidisciplinary team within eachrotation, as part of a team of pharmacists working in that speciality beingprofessionally accountable for their actions.

Provide drug-related advice andinformation to patients, consultants and other health-care professionalswithin a Clinical Management Team.

Monitor the clinical use of drugsin patients. Advising medical and nursing staff how to optimize drug therapy.Reporting prescribing and administration near misses, and risk managementissues

Provide education to healthcareprofessionals within the speciality

Participate in the training ofjunior pharmacy staff within the speciality

Support the Lead Pharmacist for thearea with the production and implementation of drug related policies andprocedures, with the production of financial reports and with horizonscanning activities.

Participate in operational servicecommitments to dispensaries, Medicines Information and aseptic services

The post holder will be required towork evenings, weekends and Bank Holidays on a rotational basis andparticipate in the on-call from home pharmacists rota.

Qualifications
  • Masters qualification in Pharmacy, or equivalent qualification, plus completion of pre-registration training
  • Pharmacist Registrant of the General Pharmaceutical Council (GPhC)
  • Postgraduate Diploma in Clinical Pharmacy or equivalent experience or qualification
