Role Overview

- Develop and deliver pharmacy services to patients at Medway Hospital.
- Ensure patients get the most from their medicines and receive optimal support.
- Rotate through Pharmacy specialties: Acute medicine, Admissions, Cardiology, Surgery, Virtual Hospital, Paediatrics, Critical Care (level 2 and level 3).
- Gain experience in Anticoagulation, Aseptic services, Governance and Risk, Research and audit, and Antimicrobials.
- Liaise between wards, departments and pharmacy to tailor services to the Trust’s and patients’ needs.

Job Responsibilities
- Antimicrobial Stewardship
- Work with the Lead Antimicrobial Pharmacist to review policies and protocols to ensure they reflect current evidence-based practice.
- Ensure adherence to antibiotic guidelines, policies and strategies.
- Assist with management of the Antimicrobial Prescribing application.
- Provide proactive input into antimicrobial management of patients with sepsis and inappropriate use.
- Engage with the Infection Management Team and attend anti‑infective meetings.
- Audit adherence to antimicrobial policies and provide feedback to divisions.
- Provide antimicrobial usage data and feedback to the Antibiotic Stewardship Group.
- Act as an antimicrobial resource for clinical staff.
- Engage with Hospital at Home and Surgical Patient Enhanced teams for safe outpatient antimicrobial management.
- Attend antimicrobial ward rounds led by consultants.
- Patient Services
- Dispense and check medication as required.
- Link clinical and patient services, supervise dispensary practice.
- Ensure prescribing policies for unlicensed and out‑of‑licence drugs are adhered to.
- Support the Dispensary Management team and reduce dispensing errors.
- Education and Training
- Serve as Practice Supervisor/ Educational Supervisor for Foundation Pharmacists and Pre‑Registration Pharmacists.
- Conduct Supervised Learning Events and ward assessments.
- Participate in the training of pharmacy staff, other professionals and pharmacy students.
- Maintain an active portfolio of Continuing Professional Development (CPD) and participate in departmental CPD sessions.
- Policy Development, Financial & Resource Management
- Update or write SOPs.
- Develop policies to control drug expenditure and ensure cost‑effective prescribing.
- Support the Lead Pharmacist in monitoring drug expenditure and providing accurate analysis.
- Identify potential cost‑savings and implement systems to realise savings.
- Educate other professionals on drug costs and promote evidence‑based prescribing.
- Assist in implementing local and national directives (NICE, Patient Safety Alerts).
- Audit and Research
- Undertake clinical audit and research in pharmacy and with other professionals.
- Implement changes from audit projects and Quality Improvement Projects.
- Publish and present findings locally, nationally and internationally.
- Supervise audit and QIP projects undertaken by students and pharmacy technicians.
- General
- Report any complaints, incidents or near‑miss situations via the error‑reporting system.
- Maintain an active CPD portfolio.
- Show confidentiality regarding patient information and Trust commercial data.
- Uphold Clinical Governance principles.
- Operate under relevant laws, RPS, GPhC, Trust procedures and professional standards.
- Undertake other duties as directed by the Chief Pharmacist.
Person Specification
Qualifications
- Master’s degree in Pharmacy and registered with GPhC as a Pharmacist in the UK (or eligible to be).
- Diploma fully complete (1 point) or certificate only (1 point).
Experience
- 18 months of post‑qualification experience within pharmacy.
- Hospital pharmacy experience.
- Evidence of MDT working.
Knowledge
- Good core therapeutic knowledge.
- Understanding of medicines management.
- Understanding of medicines safety.
Skills
- Excellent communication skills (verbal and written) with healthcare professionals and patients.
- Methodical and able to prioritise workload.
- Leading/mentoring/coaching experience.
- Problem‑solving skills.
