Qualifications

• Active Pennsylvania Registered Nurse licensure required.
• RAC-CT certification preferred.
• Gerontological Certification preferred.
• Active CPR required.
• Two-years RNAC or related experience preferred.

Responsibilities

• Oversee and coordinate the entire collaborative assessment and care planning process.
• Ensure appropriate services delivered and available to residents.
• Maintain organized reports and data using appropriate guidelines.
• Resolve insurance or managed care operational issues.
• Monitor rehabilitation therapy utilization for all residents.
