Job description

We are looking for an enthusiastic, dynamic individual, able to demonstrate a strong sense of initiative, adapting to any given situation, who works well as part of a team. As physical health Practitioner, you will be supporting the delivery of implementing bespoke intervention which support our patients with any given physical issues .

Your role will be diverse, work is varied, can be fast paced and demanding, so the post holder must be flexible in their practices.

We ask you to be committed to delivering a quality service and you are passionate in developing/improving service provision. We offer, training opportunities whilst you are working in a supportive environment within a specialised team, regular supervision and support, opportunities for continued professional development including bespoke CPD. We will ensure you have a suitable induction period, with training provided according to your needs, along with support and weekly reflective practice from an experienced MDT.

Main duties of the job

To be an autonomous practitioner supporting delivery of interventions in physical health care.

Supporting patients with long term heath conditions maintaining accurate and relevant data

To undertake physical health assessments , attendance of Recovery Reviews when required

To work with colleagues to undertake a range of health promotion activities e.g. healthy lifestyle checks, sexual health advice, weight management programmes, and signposting to appropriate services/agencies.

Responsible for the maintenance of stock control for medical and surgical equipment and consumables as appropriate.

You will be line managed by the modern matron but will build positive working relationships with a MDT who comprises of consultant psychiatrist, nurses , HCA, to other professional such a psychology , social workers to our security reception team.

Job responsibilities

Clinical

The post holder will be an autonomous practitioner working under the direction of the Nurse Consultant/Matron to carry continuing responsibility for the following duties:

  • Support the delivery of Physical Healthcare developments and projects
  • To undertake physical health assessments working collaboratively and liaise with frontline services.
  • To support effective risk management and care planning to ensure patients physical healthcare needs are met
  • To offer specialist nursing assessment and advice where required
  • To contribute to the development of the in-house physical healthcare team
  • To ensure that clinical practice is evidence based and consistent with relevant NICE guidelines and Trust policies and procedures
  • To attend CPA's and Ward rounds as needed to feedback and liaise around long term conditions and physical health needs
  • To manage site Infection Control audits, discuss reports with ward staff and support and agree actions required in the patient environment
  • Assist with the collection and analysis of relevant data for the effective management of Infection Prevention and Control
  • Participate in the feedback of infection control surveillance/audit data to the frontline teams and clinical staff on a regular basis in order that problem areas can be identified and remedial action taken.
Person Specification
Qualifications
  • Registered General Nurse
  • Post registration experience of working as a Band 6 nurse, or an experienced Band 5 nurse seeking professional development
Knowledge/Experience
  • Experience within acute and community care settings
  • Recognises the impact that physical and psychological health has upon each other and adapts ways of working to meet the needs of individual patients
  • Experience of taking responsibility of a team or designated area of work
