Job summary

Peabody is seeking a compassionate Resettlement Officer to support people leaving hospital who may be homeless or facing barriers to housing. Based in Stoke Newington, you will help customers settle, develop life skills and progress to longer-term housing with a focus on safeguarding and independence.

The role involves coordinating with hospitals, housing providers and support agencies, carrying out welfare visits and maintaining accurate records across a 37.5-hour week. DBS check is required.

Qualifications

• Experience supporting vulnerable adults in housing, homelessness or resettlement.
• Knowledge of housing management and welfare benefits.
• Understanding of safeguarding and risk management.
• Experience completing support plans and risk assessments.
• Ability to work with external agencies.
• Good IT skills including MS Office and electronic case management.
• Organised with strong report-writing and record-keeping skills.

Responsibilities

• Support customers to settle into accommodation and access benefits, healthcare, community services and other sources of support.
• Provide practical guidance on independent living, including budgeting and healthy eating.
• Work closely with hospitals, housing providers, support agencies and community organisations to achieve positive outcomes.
• Support customers to identify and move on to suitable long-term accommodation.
• Carry out health and safety checks, room inspections and welfare visits to maintain a safe and welcoming environment.
• Respond to safeguarding concerns and work proactively to manage risk.
• Maintain accurate records, support plans and risk assessments.
