Job summary

Honeycomb Group is seeking a Move On Support Officer in Stoke-on-Trent to assist people transitioning from supported housing to independent living. You will assess needs, create trauma-informed move-on plans and help with housing options, tenancy sustainment, and independence at home.

The role involves collaboration with housing providers, local authorities and community partners, and may require driving to meet clients and partners.

Qualifications

• Excellent communication skills.
• Experience of working within homelessness, housing or supported accommodation roles.
• Positive ‘can-do’ approach.
• Understanding of great customer service and customer care.
• Ability to work under pressure and manage multiple tasks and demands.
• Ability to liaise with relevant departments and partners and with statutory services.
• Understand the principles of Value for Money.
• Full UK driving licence and use of own vehicle for work.
• DBS check required.

Responsibilities

• Conduct assessments, risk assessments, inductions, and support plans collaboratively, reviewing plans monthly.
• Develop and deliver person-centred, trauma-informed move-on plans tailored to individual needs, strengths, and goals.
• Support financial readiness and explore suitable housing options for successful move-on.
• Provide resettlement support for up to 8 weeks following a successful move into a new tenancy, and up to 10 weeks of light-touch tenancy sustainment support.
• Empower customers to maintain independence, manage their homes, and access essential items through local partnerships.
• Complete timely administration, monitor caseloads, and contribute to performance reporting.

As a Move On Support Officer, you will support people who are preparing to take the next step from supported housing into independent living. Using your understanding, empathy and resilience, you'll help individuals build confidence, overcome barriers, and create a positive future in their own home.

Working closely with supported housing providers, local authorities and community partners, you will help people develop practical skills, access suitable housing options, and successfully sustain their tenancies. Your lived experience can provide hope, encouragement and a relatable voice during what can often be a challenging transition.

We also welcome lived experience of applicants who have experienced the homelessness, supported accommodation pathway, and successfully moved into their own home. If this is you, your journey could help change someone else's future.
