Are you passionate about supporting individuals with acute mental health needs as they transition from custody back into the community? We're looking for a dedicated Resettlement Broker to join our team and help shape the future of resettlement services at HMP Wandsworth.

About the Role

As a Resettlement Broker, you'll be part of a pioneering service commissioned by Oxleas NHS Trust, working closely with healthcare teams to support service users both in custody and through the gate into the community. You'll play a vital role in developing and delivering tailored resettlement plans, offering hands‑on support from group sessions to community integration.

Key Responsibilities
  • Engage with healthcare referral agencies to identify and support service users.
  • Conduct assessments and design personalised support plans.
  • Deliver in‑custody group sessions and provide community‑based support post‑release.
  • Collaborate with external agencies to ensure continuity of care.
  • Maintain accurate records and contribute to service development.
  • Recruit, train, and manage volunteers and peer mentors.
