TheAbdominal Medicine and Surgery Clinical Service Unit are looking to appoint aRenal Transplant Co-ordinator to join the existing team to provide care forpatients undergoing transplant assessment, waiting list management andparticipate in an on-call rota.
Thesuccessful candidate will work closely with other members of the MDT as wellas the wider national transplant community.In addition, they will receive full training and support in thisexciting role.

Thispost is advertised for 0.72 WTE, we would be open to further discussionregarding the hours in necessary.
Thepost holder will be responsible for ensuring effective interdisciplinary andmultidisciplinary partnership working with key services. They will play anintegral role in order to develop the strategic provision of this service.They will be responsible for the development of a clinical pathway and theoverall journey of the patient ensuring that patients are managed inaccordance with national standards and guidelines.
Theywill be responsible for managing their own caseload of patients and theassessment process, referring patients appropriately to other healthcareprofessionals, medical, surgical psychological and social.
Theywill provide and manage an expert specialist service, developing an advancedlevel of professional autonomy and accountability in the provision of careacting as an ambassador for organ donation and transplantation
Theywill participate in a 24/7, 365 days a year on call rota, receiving referralsof organs offered for transplantation from the UK and Europe taking intoaccount the donor aetiology and clinical status with the consultant on callto ensure safe, efficient and seamless co-ordination of transplant surgery.

Work within the TransplantCo-ordinator team to develop and support our service for patients who requiretransplantation within the Leeds Teaching Hospitals
Work as a highly specialisedpractitioner within the Renal Transplant Co-ordinator team usingevidence-based practice to develop objectives and standards of clinicalpractice to support the delivery of the transplant service
Continuously monitor servicesprovided to ensure that the highest professional standards are maintained andaudited.
Analyse data and present thefindings to the relevant personnel, locally, nationally, and internationallyto promote the sharing of knowledge and practice.
Ensure the most efficient andeffective use of resources available, implementing initiatives to ensure acost-effective service.
Lead on, participate in and supportothers in the application of quality initiatives with the aim of streamliningadult and paediatric transplantation for patients ensuring their assessmentthrough to transplant is efficient.
Asa team of highly specialised nurses and practitioners, participate inanalysing existing service delivery and determine future service needs inline with local and national requirements.
