• Minimum 2 years post-qualification experience.

Responsibilities

• Processing repeat and non-repeat medication requests.
• Conducting clinical medication reviews with patients.
• Supporting and implementing prescribing policies.
• Reconciliating medicines after hospital discharge.
• Monitoring prescribing patterns for compliance.
• Answering medicine-related queries.
• Contributing to audits and quality improvement.
