Job summary

We have an exciting opportunity for a Release and Transfer Lead to join our Primary Care team at HMP Brixton, a category B/C prison located in Brixton. HMP Brixton is a male prison with a capacity for around 800 inmates. This role does not require a clinical registration.

Hours

Monday to Wednesday, 0900 - 1700.

Salary

Full-time. Annual salary up to £61,119 per annum (FTE), depending on experience.

Main duties of the job
  • Management of assessments of health needs, planning, delivery and evaluation of care for the prison population, including clinical risk assessment.
  • Management of key risks associated with resettlement: disengagement/apathy, anxiety related to housing, relationships and financial concerns, newly sentenced, increased suicide/self-harm risk, detoxing on transfer, mental illness, poorly managed challenging behaviours.
  • Attend a multi-disciplinary pre-release meeting including representatives from safer custody, healthcare and external services.
  • Liaise with social care providers and with other establishments on receiving and sending patients for transfer.

Qualifications required
  • Evidence of implementation of local and national policies (e.g., NICE guidance), developing protocols/care pathways and monitoring effectiveness.
  • Experience demonstrating high levels of clinical expertise in decision-making and judgement, enabling the development, monitoring and improvement of standard of care through supervision, clinical practice, audit, research, education and professional leadership.
  • Line management experience.
