Job summary

Seetec Communities is seeking a Support Worker (Peripatetic) to join the CFO Evolution programme on a long-term Fixed Term Contract until December 2028. You will manage a caseload across custody and community settings, delivering tailored interventions and coordinating risk management to reduce reoffending while safeguarding adults and children.

You will work with peer mentors and volunteers, arranging housing, substance misuse, and health services, and travel regularly across the South West

Qualifications

• Experience supporting adults and/or vulnerable individuals within custodial or community settings.
• Ability to work with multi-agency partners to arrange housing, health and social care services.
• Strong skills in assessments, planning and delivering tailored interventions.

Responsibilities

• Manage a caseload of participants across custody and community settings.
• Conduct assessments, create individual action plans and deliver one-to-one and group interventions.
• Record participant data accurately to meet contractual and administrative requirements.
• Meet personal and team performance targets.
• Coordinate risk management and safeguarding for adults and children, liaising with Probation Services.
• Work with peer mentors and volunteers to access housing, substance misuse and health services.
• Travel across the designated cluster and attend internal/external meetings.
