Communities is hiring a Support Worker (Peripatetic) to support the CFO Evolution programme under a fixed term contract until December 2028. The role involves caseload management across custody and community settings, delivering tailored interventions and safeguarding adults and children.

You will travel across the South West cluster to deliver services and liaise with partners and volunteers. Responsibilities include risk oversight, initial assessments, and preparing participants for release,

Qualifications

• Willingness to travel across the cluster/region and work in community and custodial settings.

• Strong time management and organisational skills to manage multiple activities.

Responsibilities

• Manage a caseload of participants across custody and community settings.
• Complete pre-release work, through the gate support, assessment and tailored action plans; deliver interventions 1:1 and in groups.
• Ensure accurate recording of participant data for contractual, administrative and financial reporting.
• Meet individual and team performance targets and support risk management.
• Travel regularly across the cluster for caseload management, service delivery and meetings.
