Job description

An exciting opportunity has arisen to join the 0-19 Stoke Integrated Health Visitor and School Nurse Service delivering public health services to children and families across Stoke on Trent. We have a vacancy offering a 22.5hrs a week for a Band 6 Public Health Practitioner (HV/SN) based in the 0-19 Stoke Hub. The Hub is based in Smallthorne Health Centre and provides a single point of access to the Health Visiting and School Nursing Integrated Service utilising new and innovative technology to reach out to the Young People and Families to provide advice and support. We are seeking a person who will be caring and compassionate, who has the ability to actively listen and have a warm and approachable manner. You will lead by example and encourage effective skill mix team working to develop others and ensure the best outcomes for our Children, Young People and Families across Stoke on Trent. As a trust we are committed to supporting professional and personal development of our staff and have excellent training opportunities that align with our integrated 0-19 service. We also provide the Transfer of Fields (TOF) Training which is an opportunity to develop your knowledge and skills within the 0-5 and 5-19 elements of the service. The expectation would be that you would commence the relevant Transfer of Fields course within the first 12 months of employment.
