Description

We are recruiting a Senior Practitioner for the Adult Mental Health Assertive Outreach Team at Caerphilly County Borough Council. This role is based at Hafan Coed, Blackwood.

Working hours: 37 hours per week. Contract type: Permanent, Full Time.

Responsibilities
  • Provide assessment and care management services in accordance with National Occupational Standards for Social Work and the Code of Practice for Social Care Workers.
  • Practice in accordance with current legislation and policy, including the Social Services and Wellbeing Act 2014, Mental Health Act 1983 and related Welsh legislation.
  • Work closely with carers, other professionals, and statutory agencies (housing, police, probation, third sector) to deliver integrated care.
  • De‑saturate crisis situations and facilitate safe discharge of service users from hospital into the community.
  • Operate as part of a multi‑disciplinary team, sometimes working independently using own initiative.
  • Attend meetings using a UK Category B (Cars) driving licence and company vehicle as required.
Qualifications and Requirements
  • Diploma or Degree in Social Work or equivalent predecessor qualification.
  • Approved Mental Health Professional (AMHP) award (or willingness to undertake training to obtain it).
  • Registered as a Social Worker with Social Care Wales, or able to transfer from another recognised body.
  • Completion of the Consolidation of Practice qualification within the first 3 years of practice.
  • Understanding of relevant legislation and policy.
  • Full UK Category B driving licence and access to a motor vehicle insured for business purposes.
