Job summary

A well-established healthcare practice in Thornbury is looking for a motivated Practice Nurse with Chronic Disease Management experience to join their clinical team. The successful candidate will conduct wound care, immunisations, and cervical smears, while also providing routine nursing care to patients. Excellent communication skills and the ability to work proactively in a team are essential. A registered nurse with a diploma in COPD and experience in primary care is preferred. Qualifications

  • Experience of working in a GP practice.
  • Experience managing COPD, Asthma and/or Diabetes.
  • Experience of working within a nursing team.

Responsibilities

  • Implement health promotion and well-being programmes.
  • Manage patients at risk of developing long-term conditions.
  • Provide nursing care as per clinical-based evidence.
  • Conduct immunisations and cervical cytology screenings.
  • Maintain accurate clinical records.
  • Support the team in clinical emergencies.
